PURPOSE: This application uses questionnaires to obtain personally identifiable information (PII) from an individual so that key military personnel can assess the state of the individual's fitness for readiness purposes and before possible deployment outside the United States, its territories, and its possessions.

ROUTINE USES: In addition to those disclosures generally permitted under 5 U.S. 552a(b) of the Privacy Act of 1974, as amended, the DoD "Blanket Routine uses" under 5 U.S.C. 552a(b)(3) apply to this collection (http://dpclo.defense.gov/privacy).
